Lipoedema (also known as Lipedema) is a chronic medical condition where there is an abnormal build-up of fat cells in the legs, thighs, buttocks, and sometimes in the abdomen and arms too. This condition can often be confused with patients appearing to be obese and/or to have swollen legs and abdomen. Liposuction is a very successful technique for Lipoedema that can remove excess fat cells and may improve any associated lymphedema too.

Lipoedema is a fat tissue disorder. It is a genuine medical condition characterised by an accumulation of painful fat below the waist, resulting in hips, buttocks and legs that are out of proportion with the upper body. It can also affect the arms, resulting in laxity of skin with abnormal fat pads at the elbow and wrist. It typically affects women and often develops during puberty.
Over time, expanding fat cells can block the vessels of the lymphatic system, which normally helps return fluid to the blood stream. This blockage prevents the proper drainage of lymph fluid. Lipoedema is estimated to occur in 11% of the adult female population, meaning that millions of women worldwide are affected. A large number of health care professionals remain unaware of the condition and patients are simply dismissed as being overweight.
Liposuction is a surgical procedure for removing fat. In patients with lipoedema, liposuction can remove these fatty deposits permanently. If liposuction is performed then you will see an immediate reduction in the size of the limb. However some patients have a combination of lipoedema and lymphoedema and it is essential that they continue to wear compression garments, as the limb will swell again if the compression is not maintained. For patients who do not have lymphoedema, compression garments will not be required in the long term and the fat will be permanently removed. The input of a lymphoedema nurse is essential to the process.

Mr Ahmed Abd El Gawad is a Consultant Plastic Surgeon with over 20 year of experience within his field. Mr El Gawad trained and practised at the prestigious University Teaching hospital in Cairo, Egypt, where he learnt advanced techniques in Plastic Surgery and Trauma Surgery. After a successful 10-year career in Cairo, Mr El Gawad joined one of the greatest landmarks in international healthcare: the NHS. He has worked in renowned hospitals across the UK within the Plastics, Burns, Trauma and Lower Limb departments. At present, Mr El Gawad is the Lead Surgeon for Plastic Surgery and Lower Limb Trauma at the Royal Liverpool University Teaching Hospital.
Alongside his day-to-day practice, Mr El Gawad regularly returns to Cairo to teach and train the next generation of Plastic Surgeons and is invited internationally to speak at conferences about the research and advanced techniques he has used in his remarkable career thus far.
He is a member of both BAAPS (British Association of Aesthetic Plastic Surgeons) and BAPRAS (British Association of Plastic, Reconstructive and Aesthetic Surgeons), as well as being a Fellow of the Royal College of Surgeons (RCSEd) and the European Board of Plastic Surgery (EBOPRAS).
Whilst Mr El Gawad is highly skilled and experienced in standard aesthetic practice and cosmetic surgery – breast augmentation, uplift, reduction, anti-wrinkle treatment, dermal fillers; he has a special interest in advanced and more complex cosmetic surgeries. Since 2001, Mr El Gawad has been trained in seeing patients who suffer from lipoedema and/or lymphedema. Mr El Gawad has learnt advanced techniques (including liposuction and excision of tissue) from one of the most experienced lipoedema/lymphedema surgeons in Egypt and has since gone onto perform hundreds of procedures within Egypt and most recently within the UK. Unfortunately, the NHS does not fund lipoedema or lymphedema as they consider the treatment to be cosmetic rather than therapeutic. This does mean there are few surgeons in the UK who have vast experience of the condition and surgery itself. Fortunately, Mr El Gawad was able to gain significant experience in Egypt, under the mentorship of a leading Plastic Surgeon. During your consultation, which normally last around 20-45 minutes, you can ask as many questions as needed, and hear from the surgeon about the tumescent liposuction procedure, your recovery, what results can be achieved and any risks that may be involved.
Before any lipoedema treatment/surgery can take place, we will invite you to attend a pre-operative assessment in clinic. Here you will meet the nurse who will perform a series of checks to make sure you a medically fit for surgery. We check all patients for MRSA and may conduct some blood/urine tests too. One of the main parts of the pre-operative assessment for lipoedema is taking photographs to have a medical record before surgery.
You will have time to ask as many questions as needed, and the Nurse will be on hand to guide you through your final days before surgery including what to bring, when to arrive and how to prepare on the day of surgery.
On the day of surgery, you will attend our own private hospital in Newton-le-Willows. There is free on-site parking and on arrival after checking in at reception, you will be greeted by the Nurse who will take you to your room. You will given a beautiful and clean en-suite room for your every comfort with air conditioning, a telephone, free WiFi and free-view television. The Surgeon will also visit to check you are fit and well and ask for final consent before proceeding to theatre. Once sedated with a general anaesthetic, the surgeon will perform the tumescent liposuction procedure.
Following surgery, you will not be abandoned! You will be invited to come into clinic and see the Surgeon/Nurse regularly, typically at 1, 2, 6 and 12 weeks after surgery. This allows the Nurse or Surgeon to check your recovery and take further photographs to record the changes in your fat reduction.

You can come into clinic as many times as you want (its free of charge), we’re here to make sure you have the best possible results and a comfortable recovery. What is tumescent liposuction?
Tumescent liposuction is a surgical procedure where the unwanted fat cells (subcutaneous fat) are sucked out the body via liposuction using a tube and are permanently removed. To help numb the area first, and to help prevent blood loss, a liquid solution (with a local anesthesia) is first injected into the area to be treated.
Tumescent liposuction is very effective in treating Lipoedema. Though several operations (staggered over e.g. 6 months) may be required to remove the unwanted fat cells from the different areas of the body. Alike any liposuction procedure, if the customer gains weight after having had the surgery, the fatty swellings may return to the same level as before treatment.
What will I need to wear after surgery?
Special compression garments are usually worn for lengthy periods of time after having tumescent liposuction. This is to improve healing and prevent complications such as Lymphoedema. This is required both during the day and overnight. It is important you have some support during the first 24 – 48 hours after surgery to help you at home. You must always have a chaperone (whether friend or family) to take you home when discharged.
When can I return to work after lipoedema surgery?
This would depend on the nature of your job and how you are progressing with your recovery. It is common to require 2-3 week off work, but if you are an office worker, doing only light administrative tasks, you will be physically able to return much quicker than a person who has a more physical job. If you need to move/lift heavy items or have a very hands on job, it would be ideal to have longer for recovery period. It would be recommended to always arrange a couple of weeks of lighter tasks upon your return to work, as exertion can cause side-effects and reduce your healing/recovery.
When can you exercise after lipoedema surgery?

You can drive when you feel physically able to during your recovery. This is commonly around 1 – 2 weeks following surgery but is highly dependant on how fast you are recovering and regaining strength and suitable body control to be able to safely drive and meet the terms of your driving license.
Why Do I Need To Go Private?
The NHS does not commonly carry out Lipoedema treatment/surgery unless there is an associated and significant distress or pain. For this reason, many women and men choose to have the treatment done at a private hospitals such as Pall Mall Cosmetics.
